Carestream Dental Releases New Patient Forms

Carestream Dental has introduced its new contactless Patient Forms, which gives patients added peace of mind by giving them the option to fill out relevant forms while remaining safe at home. It also allows staff to work more efficiently.

Patients can access their forms via a link on the practice’s website, meaning they spend less time in the waiting room touching pens and paper. If a patient hasn’t completed their forms ahead of time, a secure link can be shared to their phone or tablet for quick and convenient updates.

Patients Forms is integrated with Carestream’s practice management platforms. A patient’s information is automatically written back to their chart, saving time for front desk staff. Practices also have the option to customize Patient Forms with practice logos and other information outside of the templates provided, including a COVID-19 screening form.

Plus, by eliminating common in-office tasks at the beginning of the appointment, practices can have more time to disinfect equipment at the end of the appointment and prepare for the next patient.”
