How to deliver optimal care with the right restorative essentials

In this case, a 32-year-old male presented with an existing large mesiooccluso-disto-lingual amalgam and fractured mesiobuccal cusp on tooth No. 14. The tooth was otherwise sound, so an overlay was indicated, and the tooth was conservatively prepped with minimal reduction of about 1.5 mm on the occlusal surface, and the patient was sent home with a temporary. A lab-fabricated overlay was fabricated from KATANA Zirconia STML™ from Kuraray Noritake

When the patient returned 2 weeks later, the temporary was removed.

To ensure an optimal surface for adhesion, air abrasion with the PrepStart H2O Hydro Abrasion System from Zest Dental Solutions with 27-µm aluminum oxide removed any remaining cement, biofilm, and debris from the preparation.

The preparation was next coated with a 35% phosphoric acid solution to clean any residual resin coating and aluminum oxide on the dentin. This also etched the exposed enamel.

Air abrasion was now performed on the overlay to clean up the internal substrate and create a microscopically roughened surface to enhance retention of the bonding agent.

CLEARFIL CERAMIC PRIMER PLUS from Kuraray Noritake was applied to the internal surfaces of the overlay.

PANAVIA™ V5 Tooth Primer from Kuraray Noritake was then applied to the preparation for 20 seconds and air thinned, with no need to light cure. The primer’s touch-cure technology ensured that once the resin cement was introduced to the primer, polymerization began immediately on the preparation and the restoration interface.

PANAVIA V5 resin cement from Kuraray Noritake is directly injected onto the bonding surface of the overlay.

The restoration is seated onto the prep and tack cured for 2 to 3 seconds. The overlay is held gently in place with a composite packing instrument while excess cement is peeled off with a curette and while flossing the interproximal spaces.

After cleaning and flossing, a final light cure is performed: 20 seconds on the buccal, 20 seconds on the lingual, and 20 seconds on the occlusal.

How to Increase Efficiency, Predictability, And Durability In Class I Restorations

An 18-year-old female patient presented to the dental practice for a routine hygiene appointment and checkup. During a thorough clinical exam, probing detected “stickiness” on the occlusal surface of tooth No. 18, which was verified as isolated Class I occlusal caries.

The agreed-upon treatment plan involved removing the caries using a conservative and minimally invasive approach to maintain the integrity of the tooth structure and retain as much enamel as possible for a durable, long-lasting restoration.

The tooth would be restored using a tooth-colored composite material. The patient was anesthetized, and the site was isolated using a rubber dam.

A carbide bur was used to remove the carious lesion followed by a diamond bur to smooth and refine the angles of the preparation.

With the preparation design complete, the phosphoric acid, Total Etch from Ivoclar Vivadent, was applied to the enamel for 15 seconds and dried using the Warm Air Tooth Dryer from A-dec.

After the preparation was thoroughly rinsed, Adhese Universal was applied, using the Adhese Universal VivaPen single unit dose applicator.

The adhesive is then air dried using a warm air dryer.

Next the adhesive was light cured for 3 seconds using the Bluephase PowerCure, a highintensity LED curing light from Ivoclar Vivadent.

Now, the first layer of flowable direct composite—Tetric PowerFlow from Ivoclar Vivadent—in shade IVA was placed as a dentin replacement

It is then cured for 3 seconds using the Bluephase PowerCure. This flowable composite is used to coat the internal aspects of the cavity and to ensure a complete seal against microleakage and recurrent caries.

During polymerization, the initial visual high translucency gradually changes to a more opaque shade that is close to that of natural dentin in value. The material’s final opacity conceals the stained underlying tooth structure to optimize the esthetics of the final restoration.

After the flow layer is placed and cured, a layer of Tetric PowerFill bulk fill composite from Ivoclar Vivadent in shade IVW was placed in a single 4-mm increment. It is sculpted using a contouring instrument and light cured for the manufacturer-recommended 3 seconds.

Once curing and occlusal adjustments were made, the restoration was finished and polished using a silicone polisher to create the final occlusal restoration seen here.

Technique in 2 Minutes: How To Use The Universal Bond System For A Perfect, Predictable Fit

A view of the mandibular ceramic restorations on the master model before cementation. The restorations are fabricated from IPS e.max from Ivoclar Vivadent.

First, each restoration is etched with 9% hydrofluoric acid Porcelain Etchant from BISCO for 30 seconds.

Next, the hydrofluoric acid etchant is rinsed thoroughly from the inside of the restorations, then air-dried.

In the next step, a phosphoric acid etch such as Ultra-Etch from Ultradent, is placed on the preparations and after 15 seconds is rinsed off with water for at least another 15 seconds. Next, the preparations are thoroughly air-dried.

Now Consepsis 2% chlorhexidine solution from Ultradent, is used to disinfect the preparation,

Before the excess chlorhexidine is removed using a high volume suction so the tooth structure remains hydrated.

With the preps etched, rinsed, and cleaned, Tokuyama Universal Bond is then applied to the preparations.

The bonding agent is air thinned, which is done to evaporate the solvent.

After placing a coat of Tokuyama Universal Bond on the inside of the restorations, they are then loaded with EsteCem II resin cement from Tokuyama Dental America.

The mandibular incisor restorations are being held in place and positive pressure is applied to express excess cement around the periphery of the margins.

A No. 2 Keystone flat brush from Patterson Dental is bent at a 45-degree angle using a hemostat. It is then used to remove excess cement from the marginal areas of the seated restoration.

Next, the restorations are tacked to place with a curing light so the remaining excess cement can be cleaned away without dislodging the restorations.

Dental tape is now used to remove cement excess from the interproximal areas.

Once cement removal is complete, the restorations are light-cured according to the manufacturer’s instructions.

Here is a facial retracted view of the cemented mandibular ceramic restorations.

Here, a protrusive excursion shows separation of all posterior teeth.

A right lateral working excursion displays separation of all posterior teeth on the patient’s right side.

A left lateral excursion shows separation of all posterior teeth on the patient’s left side. Note that because of the post orthodontic positions of the teeth, the cuspids do not line up. Anterior coupling of the remaining anterior teeth protects the posterior teeth during the functional movement.

The case finished, here is a retracted full arch view of the completed cementation in centric occlusion.
