Sidebars: Dental Adhesive Know How
Types of Bonding SystemsMulti-bottle, total-etch wet bonding systems consist of a phosphoric acid etchant, which after being rinsed off, is followed by a hydrophilic primer and a hydrophobic adhesive. A wet bonding approach is used. Examples include OptiBond Fl (Kerr), All-Bond 2 (Bisco) and Scotchbond Multipurpose Plus (3M ESPE).
One bottle, total-etch systems also use a phosphoric acid etchant, but then follow up with a combination of the primer and adhesive from one bottle. A wet bonding approach is also used. They are not better or faster than multi-bottle systems, but are easier to use because fewer bottles are involved. Examples include Prime & Bond (Dentsply Caulk), OptiBond Solo Plus (Kerr) ), PQ1 (Ultradent) and One-Step Plus (Bisco).
Two-step self-etching bonding systems use a self-etching resin primer in place of the traditional phosphoric acid etchant followed by the application of an adhesive. Examples include Clearfil SE Bond (Kuraray) and Tyrian SPE (Bisco).
One-step or “all-in-one” self-etch systems combine primer and adhesive (either from two components or one) into one final material. Examples include Adper Prompt-L-Pop (3M ESPE) i-Bond (Heraeus Kulzer) OptiBond All-in-One (Kerr) and Xeno IV (Dentsply Caulk).
Common Mistakes
While they’re considered the “gold standard,” total-etch materials are still somewhat technique-sensitive. Here are some of the most common mistakes clinicians make when using these materials:
Not using a wet boding technique properly. The dentin must be correctly hydrated when applying primer. The dentin should exhibit a glistening appearance before the primer is applied. If the tooth is too dried out it makes it more difficult to penetrate to the depth of the etch, translating into poor clinical performance.
Overetching the dentin. Although this is not a big problem, phosphoric acid is much stronger than the acids used in self-etch materials and should be rinsed after a 15-second application with most total-etch adhesives.
Inadequate primer application. Primer contains volatile solvents like ethanol and acetone. To adequately seal the open dentinal tubules after acid etching, sufficient coats of primer need to be applied. If you don’t place enough primer, you may not seal the tubules and that
